I agree to waive any and all rights, claims, damages, action, causes of action suits of any kind or nature, and give permission to be photographed and/or videotaped while participating in this event. These photos and/or videos may be used for future promotion of recurring or similar type of event.                       

I understand that I am responsible to discuss my concerns about any health or physical problems with my physician, my artistic director(s) and/or dance instructors prior to participation. I assume the risk if unexpected events may occur and result in harm, injury or illness to me or damage to my property during my participation in this event, I agree to indemnify and/or release ASU Gammage, Arizona Dance Coalition, Desert Dance Theatre and Dance Theater West from any and all claims, actions, and demands.
