UCSC Community Impact Awards: Faculty Community Impact Award
Each year, UCSC recognizes the extraordinary commitment of students, staff, faculty,  and community partners in supporting the community engagement of UChicago students. UCSC will honor all recipients of the aforementioned awards, as well as the Perry Herst Prize and President's Volunteer Service Awards, at a reception on Wednesday, May 8.  We encourage you to nominate those who are deserving of this recognition. This form is open to all members of the University of Chicago community, though we particularly encourage students to submit nominations. 

The Faculty Community Impact award recognizes the extraordinary commitment of a faculty member in either direct service or in the support of students engaged in community service and civic engagement. Previous award winners have been recognized for their integration of community service and social justice-related activities into their teaching, for engaging or supporting students in community-based research, and for their sponsorship of RSOs and other campus-community partnerships.

Nomination Submission Deadline: Friday, April 26, 11:59pm.
