UCSC Community Impact Awards: Outstanding Community Mentor
Each year, UCSC recognizes the extraordinary commitment of students, staff, faculty,  and community partners in supporting the community engagement of UChicago students. UCSC will honor all recipients of the aforementioned awards, as well as the Perry Herst Prize and President's Volunteer Service Awards, at a reception on Wednesday, May 8.  We encourage you to nominate those who are deserving of this recognition. This form is open to all members of the University of Chicago community, though we particularly encourage students to submit nominations. 

The Outstanding Community Mentor award recognizes the extraordinary commitment of a community organization staff member who engages students in their mission and educates them about the social justice issues to which they respond. Previous award winners have mentored students as volunteers, hosted interns, and partnered with RSOs and other student-led organizations to create and co-sponsor social change initiatives throughout Chicago. 

Nomination Submission Deadline: Friday, April 26, 11:59pm.
