Please complete this form to sign up for a Spring 2024 OHCHS athletic team. All paperwork must be submitted prior to the first practice on Monday, March 25.  All students must have a completed IHQ form. If you previoulsy participated in a Fall or Winter 2023 - 2024 sport,  you have already completed an IHQ.

All students participating in school athletics must have some form of health or accident insurance. If you need to purchase insurance to cover your student during the athletic season, here is the link for the school insurance:  www.1stAgency.com 

All student athletes must have a up to date (less than two years) physical exam on file in the school's Health Office. If the physical expires (more than two years from the date of the physical exam) during the season, the student athlete will become ineligible to practice or participate. The student should have an updated one on file prior to it expiring. Updated physicals can be faxed to: 207-805-7923. 

Students handing in paperwork at the first practice will not be allowed to practice that day. All students must be cleared through the Health Office prior to being allowed to practice.
Spring season practices for Baseball, Boys Lacrosse, Girls Lacrosse, Outdoor Track and Field, Softball, and Girls Tennis will begin on Monday, March 25th.
