All student athletes must fill out this annual form (after August 1, 2023) and submit a current physical to the Health Office directly or fax it to (207) 805-7923. Physicals are current for 2 years from the exact date of your last physical. Before starting this form please have your emergency contact information, medical history, and insurance information ready. If you need to purchase insurance the district offers athletic related insurance plans at www.1stAgency.com.
