Attendance Confirmation for In-Person Worship
*This is a WEEKLY attendance form to complete to confirm your attendance for the upcoming "Worship at Lawn", Sabbath vespers held every Saturday @ 6: 30 pm.

*This form is for households to complete who do not wish to receive the weekly attendance check via TEXT msg.  You must complete this form EVERY FRIDAY by 12 NOON to confirm your attendance the following day for Sabbath vespers.

Screening Symptoms Questions:
Your safety and health is a top priority during our in-person worship.  If any members in your family have the following symptoms within the last 14 days PRIOR to attending in-person service, please REFRAIN from attending until you have cleared 72 hours without symptoms.  These are the following potential COVID symptoms:  FEVER, CHILLS, BODY/MUSCLE ACHES, COUGH, SHORTNESS OF BREATH, FATIGUE, HEADACHE, SORE THROAT, NASAL CONGESTION, LOSS OF TASTE/SMELL, NAUSEA, VOMITING, DIARRHEA.
Checking In Process
1.   Upon arriving at church, you will be screened with a symptom questionnaire, temperature, & mask check.
2. Then, you will be escorted to your seating area and attendance verified.
A friendly reminder of items to bring:
1. Bring blanket or camp chair for your seating.
2.  Water to stay hydrated
3.  Hat/Umbrella for shade
4. Wear Sunscreen
