Thank you for your interest in participating at the 2022 Cultural Day Celebration hosted by Diversity & Equal Opportunity Network Inc.- (DEON) in partnership with Evesham Township and their Human Rights Advisory Committee.

**Parent/ Guardian must complete this form for under age of 18.

•Please fill out and submit this form to be added on our contact list.
•You will be contacted by someone from DEON to confirm your application or you may email sayhello@deon4change.org for more information and questions.

Should you need to submit music to the DJ please do so, that all performance tracks be emailed to Dj Harold Bishop at djbishop@djbishop.com NO LATER than Thursday, October 8th to ensure that the DJ has them on deck for the event.

***Should the weather not be in favor the location of this event will move to The Blue Barn-  1004 Tuckerton Rd, Marlton, NJ 08053. You will be notified within 24 hours of the event should a venue change occur.
