Thank you for your interest in participating in the 2022 Cultural Day Celebration in Marlton, hosted by The Diversity & Equal Opportunity Network- (DEON) in conjunction with Evesham Township and their Human Rights Advisory Committee.
PLEASE READ THIS SECTION & SUBMIT THIS COMPLETED FORM.

COST: FREE
DATE: October 8th 2022
TIME: 11am- 3pm
LOCATION: Evesboro Downs Park (131 Evesboro-Medford Road, Marlton, NJ  08053)
SETUP FROM: 9:00am-10:30am; No cars will be permitted on the field after 10:30am for set-up.

As a Food Truck/Food Vendors are required to the following:

1. All Food Truck/Food Table Vendors will also need to contact the Fire Marshal’s office to discuss all of the
following that may be needed in order to attend the event:

A. Fire Permit (normally a fee of $54, due prior to the event)
B. Fire Marshal Inspection (done on the day of the event)

2. Hold Harmless Agreement (see below or https://drive.google.com/file/d/1eFLmN8xId4DgE6xP72glqJPTKofvtoU-/view?usp=sharing) must be completed and returned via email to Joana Takacs
(TakacsJ@evesham-nj.gov).

3. Please provide a Liability Insurance Declaration Page/Certificate of Insurance (proving dates of service for
the event, $1million Commercial General Liability limits, $1million General Aggregate limits and $1million
Products Completed Operations Aggregate; they will also need to list Township of Evesham as an “Additional
Insured”)

4. Burlington County Health Permit (if you have a health permit from a different county, you will need to contact
Burlington County directly to get one from Burlington County). The contact is George Hamway 609-265-5565

*Please note: in addition to the above requirements, any table vendor that is selling any food requiring
heating/sterno burners will also need to have a tent that you are serving the food under. Additionally, you will
need to bring you own table, chairs and tent (10 X 10) for the space allotted.

NOTE: By submitting this application, each vendor is accepting full responsibility for his or her own entries, including loss by breakage, theft, fire, rain, wind, or any other cause of damage to inventory or person. Vendors are responsible for any insurance they deem necessary; vendors hereby expressly release the Evesham Township and Diversity & Equal Opportunity Network (DEON) from any and all liability whatsoever.
