For tracking of incidents of operations which are creating noise disruptions around the Hāna District typically caused by Bell Boeing V-22 Osprey US Military Aircraft please fill in and share this EMR tracking survey. Reporting of input to this form will on the agenda at the monthly EMR meeting.  If a Question has a red * it is required, additional information is optional. 

Any personal data will not be shared publicly – just dates, times, duration of the incident, and locations. Email is required - but please let us know if you do/do not want to be added to the EMR Mailing List.
