填寫此網上表格前，請先參閱以下注意事項 。
注意事項：
  1. 現只接受申請 2024年12月31日或以前的預約日期。
  2. 堂區辦事處將於14個工作天內以電話或電郵將預約結果通知申請者。
  3. 如申請被接納，申請人及使用者應遵守「聖堂參觀/ 導賞服務 /場地借用規則 」及填妥相關申請表格。
  4. 是次申請收集所得的個人資料將作預約用途。
Please read the following notes before completing this Google form.
Notes:
  1. Booking date on or before 31 December 2024 will be accepted.
  2. Parish Office will notify the applicant of the booking result by phone or email within 14 working days.
  3. If the application is accepted, participants are required to observe the Rules for Participation in Church Visit/ Guided Tour Service/ Venue Booking and complete the relevant application forms.
  4. All information collected here will be used for booking purpose.
