Access Card Re-Activation
SPHOA is upgrading our access system in the next few weeks and we are taking the opportunity to audit our currently issued cards.  To have your current card(s) activated on our new system you MUST complete this form.  If you have lost your cards (replaced at a cost of $25 per card), or were never issued cards, you may request new ones to be picked up by appointment at the Creative offices or at the community clubhouse during times to be determined and announced via email.

By completing the form below, I acknowledge and agree to the following:  In consideration for the privilege of using the swimming pool and facilities of Settlers Park Homeowners Association and other good and valuable consideration, the undersigned hereby elect to, and do, assume all risk for claims heretofore or hereafter arising from the subject of this Release, in favor of the undersigned, his heirs, executors, administrators, successors or assigns, and hereby knowingly and voluntarily expressly release said Settlers Park Homeowners Association, its agents, officers, employees or aides from all liability or claims, demands and costs for or arising out of injuries or damages sustained while using the swimming pool and facilities of Settlers Park Homeowners Association, or by the negligence of the undersigned.  Further, the undersigned agree to abide by and be bound by the rules and regulations for the operation and safety of the swimming pool and facilities of Settlers Park Homeowner Association.  The undersigned has read this release and understands all its terms. The undersigned executed this Release voluntarily and with full knowledge of its significance.
