This is the nomination form for individuals wishing to enter the Mens & Womens Masters carnival at the Albury Hockey Centre from Saturday April 8 - Monday April 10 (Easter), 2023.

There are three age divisions to nominate in: 40+, 50+ and 60+

The entry fee for a team is $550 (incl GST) and this will be pro rata for the team.

This form must be completed and submitted to HAW before Sunday January 29 2023.
