個人資料蒐集告知及同意書 Information Privacy Act notice and agreement
因川復有限公司（以下簡稱「TMMA古亭館」）將於您報名TMMA格鬥體驗班使用您的個人資料等，為落實個人資料保護法等相關法令遵循及保障您的權益，請您詳閱以下事項：
一、蒐集之目的：
TMMA古亭館致力將格鬥運動帶入大眾的生活，推廣真實武術的精神，開辦格鬥體驗班提供有興趣之民眾學習基本拳腿技巧，藉以達到健身、塑身、防身的效果。
二、蒐集之個人資料類別：TMMA古亭館將於報名頁面內蒐集您的個人資料，包括但不限於姓名、身分證字號（或護照號碼）、手機、電子信箱等聯絡方式，詳如報名頁面中之個人資料欄位。
三、利用期間、地區、對象及方式
(一) 期間：TMMA古亭館合法設立存續期間。
(二) 地區：TMMA古亭館及其他各館(忠孝館、天母館、雙和館
、內科館及三重館)。
(三) 對象：TMMA古亭館執行格鬥體驗班時，相關工作人員將利用之。
(四) 方式：使用於格鬥體驗班之必要聯繫、確認身份及提供相關
TMMA古亭館或其他各館之相關服務與資訊。
四、您可以針對您的個人資料請求查詢/複製/補充或更正/停止蒐集、處理或利用/刪除，請透過
TMMA古亭館 聯絡資訊與我們聯繫。
五、您得自由選擇是否提供個人資料，惟若拒絕提供相關個人資料，TMMA古亭館將無法進行必要之處理作業，您可能無法提供格鬥體驗班之相關服務。
六、若您所提供的個人資料，經檢舉或
TMMA古亭館發現不足以確認您的身分真實性或有冒用、盜用、資料不實等情形，
TMMA古亭館有權拒絕對您提供服務。

Chuan Fu Ltd (Abbr. TMMA Gu Ting Branch). Please carefully read the below, it is an agreement under the information privacy act that the personal information you provide will abide to the information privacy act of the Republic of China, Taiwan.
Below terms:
1. Purpose of the information: TMMA Gu Ting aims to bring martial arts in to everyone's lives, and to promote the spirit of martial arts. Through this trial class, we will provide a rudimentary introduction to the basic technique of the respective martial arts, to offer fitness, body sculpting, and self defense effects.
2. Types of informations: TMMA Gu Ting includes but not limited to full name, ROC identification number/passport number, mobile number, e-mail, details information which is inside the registration form.
3. Time, Region, Target person, and use of information.
(一) Time: TMMA Gu Ting legal establishment continuation use of time.
(二) Region:
TMMA Gu Ting and other branchs (Chung Hsiao, Tien Mu, Shuang He, Nei Hu Park branch and San Chong branch)
(三) Target use: Gu Ting during Trial class times, related personnel will be handling such information.
(四) way of use: during and for the communication, and to verify identification and to provide information to other branch for related services and information.
4. You Can specifically request, copy, add, change, or delete such information by contacting TMMA Gu Ting Gym.
5. If you choose to not provide such personal information, TMMA Gu Ting Branch will not be able to further process your registration, and will not be able to attend the Trial Class services.
6. If the information you provide, after realization TMMA Gu Ting is not able to verify your identification, or incorrect/false identity, TMMA Gu Ting Branch reserves the right to provide any services.
