I understand the following agreement is a condition precedent to and is only consideration for the privilege of participating in the Joseph's Playhouse, hosted by MSCBC, for the year 2023.

BEHAVIOUR OF CHILD(REN)
I understand that in order to participate in this program, my child(ren) must abide by the rules and codes of conduct established by the program staff. Joseph's Playhouse reserves the right to dismiss a child from the camp due to that child's disruption of the program, including but not limited to verbal and physical aggression against staff or other participants, failure to follow basic safety, and any other disruptive behaviour. A child's dismissal will be at the discretion of the camp director. If a child is dismissed from the Camp, there will be no refund. I also agree to reimburse MSCBC for any property damaged caused by my child’s deliberate act.

RISKS FROM PHYSICAL ACTIVITIES
I understand that the activities in this camp program include certain physical activities. I understand and agree to assume any and all risks associated with the Joseph's Playhouse activities on behalf of my child(ren). In order for my child(ren) to participate in the camp, I am willing to assume these additional risks on behalf of my child(ren).

ILLNESSES INJURIES, AND/OR ACCIDENTS
If any illness, injury, or accident occurs which, in the sole judgment of Joseph's Playhouse staff, requires immediate medical attention, I give consent for any member of the Joseph's Playhouse staff to obtain such emergency treatment including but not excluded to x-ray examination, anesthetic, blood transfusion, medical, dental or surgical diagnosis of treatment, and hospital care that may be necessary for my child(ren)'s treatment. I further consent to the signing of any releases by any and all staff of the Joseph's Playhouse, including MSCBC staffs & volunteers, which may be required by any medical care provider. I understand that in the event of an emergency medical situation I will be notified as soon as possible using the contact numbers I provided. I accept responsibility to pay for any and all financial obligations incurred as a result of any medical assistance or treatment provided in connection with any injuries or illness that my child(ren) may sustain during their time at the Joseph's Playhouse

RELEASE AND WAIVER OF LIABILITIES
For the sole consideration of the Joseph's Playhouse allowing my child to participate in this program, I hereby release and forever discharge MSCBC, the hosted facility of the Joseph's Playhouse, and all of its' staff, volunteers, and directors from any and all claims, demands, rights and causes of action of whatever kind that I may have either arising from or by reason of any personal injury or property damage resulting from or in any way connected with my child's voluntary participation in this program.

I further covenant and agree that for the consideration stated above I will not sue the Joseph's Playhouse, nor MSCBC, nor any of its' staff, volunteers, and directors for any claim for damages arising or growing out of my child's voluntary participation in this program.

I understand that the acceptance of this release, waiver of liability, and covenant not to sue shall not constitute a waiver, in whole or in part, of sovereign or official immunity by said Joseph's Playhouse, MSCBC, and all of its' staff, volunteers, and directors. I agree that if any term or provision of this release and waiver of liability shall be held illegal, or unenforceable, or in conflict with any law governing this release, the validity of the remaining portions shall not be affected.
