Star Academy Summer Program Registration
We are VERY EXCITED to announce, for your pre-planning pleasure,
specially formulated Summer Program options for your students.

Educational Activities will include:
STEAM (Science, Technology, Engineering, Art, Math)
Inventions | Origami | Exploring |Robotics | Arts and Crafts
Escape the Room Mystery | Fun with Math | Russian | Carnival
Cooking | Jewelry Making | Soap Making | Legos | Field Day

Recreational Activities will include:
Sports (Soccer, Gymnastics, Dance) | Nature Walks | Structured group games | Sprinklers |Outdoor recess

Also Included:
Two hot meals per day | One snack per day | All supplies for the daily projects

After Care:
*Due to popular demand, we are adding aftercare as an option to our day.  Aftercare lasts until 5:45pm and costs $30.00.  Students will have structured free time during after care.  Students must be picked up by 5:45pm.

Program Campuses:
At this time our Watertown campus will be housing all summer program activities. Please ask other families about car pooling each day. Watertown has an area for daily sprinkler usage, fields, air conditioning, and more!

Registration:
Registration is online this year. Registrations are first come, first registered.  It is possible that we can host multiple groups of students.  Groups are limited to participants based on COVID-19 restrictions.  To be registered we must receive:
1) Online registration filled out in full (one registration for each child)
2) Check or Venmo for the full amount (if you sign up for multiple weeks, all weeks must be paid in advance to hold spots)
Note: You may sign up for additional program weeks later, if they are available. But we cannot hold program spots without payment.

Please sign up for multiple weeks if you might be looking for exciting childcare during these weeks!

Summer Program is $400.00 per week and runs 8:30am to 3:30pm Monday through Friday.  

A typical day will include four individual activities for each student. We serve breakfast, lunch, and snack during this time. Some examples of program theme weeks last year: "A Day at the Beach," "Technology Week," and "Nature Week."

Some examples of daily activities: faux jellyfish in a bottle, seashell frames, make and fly kites, nature paintings, sun-based photography, footprints in the sand and the highlight an Escape Room for mystery week!

Students will be split by age group. All COVID-19 protocols, size restrictions, etc. will be in place unless Massachusetts changes its reopening plans before summer.  We will alert you of all guidelines and restrictions that might impact your child's summer program experience.

Summer Program Weeks (Please choose all programs you are ready to sign up for. All sessions must be paid in full to hold the program week). This registration is not complete until Star Academy has received payment in advance of the program weeks. Program weeks that are not making a minimum enrollment might be cancelled before the week runs.  We would alert you as soon as possible if that is to happen. Week of June 28 - July 2 is completely filled. *

Medical Release: Insurance Policy Number, Name of Health Insurance Provider, Primary Physician and contact for the provider. We will also need to know if there is a preferred hospital in case of emergency. *

Is your child being treated for any injury, sickness, or are they taking any medication? *
I understand that I need to pay for my program week by check by delivering to the school, or by paying as "Friends and Family" on Venmo (Star-Academy) before my program registration is complete. *

By completing this form, this parent authorizes Star Academy to render first aid for illness or injury to the child mentioned on this form.  In the event of a medical emergency, the Parent authorizes the School to arrange for transportation of the child to a hospital/ emergency care center of the school's choice. This could be for emergency medical or emergency surgical treatment.  The parent authorizes the release of any and all information including medical information to all medical personnel and treatment facilities. The Parent agrees to be solely responsible for the payment of all expenses incurred. Payment for any such expenses is due within seven days after the School's presentation to the Parent of invoices or other documentation of such expenses.  The parent releases, holds harmless, and agrees to indemnify Star Academy and its officers, agents, employees, and representatives from any and all liability, claims, losses, or expenses arising from personal injury, death, or loss of or damage to property. *
I agree to allow photographs of my child throughout the summer program experience so that we may enjoy these photos as a family.  Occasionally some photos may be selected to post to instagram or facebook for marketing purposes only.  My child will not be identified by name in any of these posts. Parent hereby waives any royalty or compensation in connection with, and absolves Star Academy from all liability related to the use of the photographs, video, or audio recordings, or other media materials featuring or depicting the child. *
I hereby give permission for the transportation of my child for official Star Academy Summer Program activities by modes of transportation agreed to by program organizers. (Transportation to parks, swimming facilities, open fields for play activities only if COVID-19 safety precautions can be secured). *
Star Academy and it's co-organizers are not responsible for lost or damaged personal property. All scheduled events are subject to change. I understand that no fees will be refunded or transferred unless a child is unable to participate due to an accident or illness per physician orders.  The undersigned parents agree to release and hold harmless Star Academy, its agents, and employees from all claims, damages, or other liabilities for injuries to student which are not the result of gross negligence by the Star Academy, its agents, or employees.  This contract shall be interpreted in accordance with the laws of the Commonwealth of Massachusetts. *
