2022-2023 The Sagemont School: Required Lower School Forms   
In an effort to continue to support the environment as well as safety and health guidelines we have gone paperless for our Back to School Forms. These digital forms must be completed in order for your child(ren) to be assigned to a class. Please read through each form carefully and make sure you have answered all applicable questions. This online form should be completed and include information for all of your children (if applicable). There will be a demographic and health section PER child. If you only have one child at the Lower School Campus, you can skip to the Consent and Release section.  

Please note for students in preschool through kindergarten, a copy of your child's birth certificate is REQUIRED. Please email copies of your birth certificate to Blanca at bherrera@sagemont.com.

If your child is NEW to Sagemont (any grade) OR is entering in preschool, pre-kindergarten or kindergarten we will need to have ORIGINAL updated Health Forms provided by your child's physician.  They will need Immunization Form #680 and Student Health Examination Form #3040. Please make sure to have these ready to submit by Friday, August 19th at the Lower School Orientation.

Your child will not be permitted to start school without these updated forms.
