Event Information Form for 21-22 Season

If you are planning on hosting an official or sanctioned event next season and haven’t been contacted by USQ already in regards to the event’s status and tentative date, please fill out this form so that we can notify you when more information is available on the status of return to play and other important updates.

Note that filling out this form doesn't replace the process of submitting your event to the USQ calendar. We will begin taking calendar submissions later in the summer. TD certification will also open up in the summer.

Please provide information on the kind of event you're running - official, unofficial, and/or sanctioned - along with how many teams you're planning for. For example, "An official event with 10-12 teams that will take place on a single day."

Event Location *

Please list city and state if you know it. If that is still being finalized, put your best guess (IE Northern California or Ohio).

Event Date *

Please list the date if you know it. If you don't have that determined yet, put your best estimate (IE fall 2021 or March 2022).
