I have voluntarily enrolled in a structured exercise program offered by CHANGE UNCHAINED FITNESS BOOTCAMP.

I have been informed and understand that there are certain risks of accident, illness, injury, or death that are inherent in any program of physical exercise. Common risks include increased pulse and respiration rate, joint stress on weight-bearing joints, fatigue, possible blood clotting which may cause fainting, sore and/or pulled muscles, side cramps, muscle cramps, and dehydration, as well as broken bones and heart failure.

I further understand that risks can be intensified and possibly severe in participants with a certain high level of risk factors identified as high blood pressure, obesity, diabetes, high body fat levels, heavy intake of alcohol, cigarette use, history of heart disease, and history of joint diseases, especially back and knee disorders.

I have been informed and understand that CHANGE UNCHAINED FITNESS BOOTCAMP, its agents, employees/sub-contractors, and affiliates have no responsibility to detect factors that may increase illness or injury for particular participants in this exercise program and that I should consult my physician before beginning any exercise program. By signing below, I consent to assume the risks which are associated with or which may result from vigorous physical activity.

In my consideration of the benefits, I will receive from the exercise program, I, for myself and my heirs, release CHANGE UNCHAINED FITNESS BOOTCAMP, its agents, employees/sub-contractors, and affiliates from any and all liability for illness, injury, or death which may occur from, or arise out of, participation in the exercise program, from any cause whatsoever, including, the negligence of those being released.

I hereby irrevocably grant to CHANGE UNCHAINED FITNESS BOOTCAMP, and its representatives, licensees, designees, permits and assigns, the right and authority to film, videotape, photograph, and/or record, in any manner or medium, me in connection with any event, and to use such film, videotape, photography and/or recordings (Products) in perpetuity, any number of times, for any purpose and in any manner or medium now or
hereafter known in connection with the display, distribution, promotion, marketing or sale of Products by any means and through any channels, including, without limitation, in or on any website, marketing, advertising, promotions, and packaging relating to CHANGE UNCHAINED FITNESS BOOTCAMP. I understand this is a legal document, and I hereby affirm that I have read, fully understand, and agree to the consent and release and terms of the service.
