Please fill out a NEW FORM FOR EACH STUDENT and only parents of students at Forsyth School should fill out this form. Only open to current students of Forsyth Schools.

We do not guarantee Christmas Assistance. By filling out the form you are giving us information to try and find assistance. The Forsyth School District does not provide holiday assistance. The school counselors receive donations from other families in the district & community on a year to year basis. There is no way to tell how many families can benefit each year.
