Touch-A-Truck Vehicle Commitment Form
This form serves to confirm your commitment to participate in COGNITION’s Touch-A-Truck fundraiser to be held on Saturday, September 21, 2019 from 10am – 3pm.

Information about where to park and who to contact on the day of the event will be communicated during the week leading up to September 21st. A representative of the owner/operator of the vehicle must remain with the vehicle at all times during the event and must complete a volunteer form.

Return a completed Vehicle Commitment Form FOR EACH VEHICLE and a Volunteer Form for each attendee by Monday, September 1st along with photo(s) and dimensions of the vehicle(s). Forms should be submitted to info@cognitiondavie.com or by mail to COGNITION Davie P.O. Box 816 Mocksville, NC 27028.
