Rehearsal Absence Request Form

Please fill out this form for EVERY absence from a rehearsal. If you are absent from multiple rehearsals, you must fill out this form for EACH rehearsal missed. This form should be filled out at least 48 hours in advance to be eligible for make-up credit (when applicable). If there is an emergency, this form must be filled out within 24 hours of the missed absence to be eligible for make-up credit (when applicable). If no form is filled out at any time, make-up work will not be available, and the student will receive a zero for the rehearsal. Please note that by simply filling this form out, you are only ELIGIBLE for make-up credit, but are NOT guaranteed it. Marching Band Students - multiple absences will result in you being placed on probation until missed information is learned and performance ready.
