Physical Activity Readiness Questionnaire
Taking part in physical activity is safe and beneficial for most people. However, some people should check with their doctor before increasing their physical activity levels. Please complete this short questionnaire below as it will tell you if you should check with your doctor before you begin training. If you are over 69 and you are not used to being physically active, you must consult a doctor in any case.

If you answered YES to one or more questions, please talk to your doctor before you engage in additional physical activity and inform your doctor of this evaluation and the questions to which you answered yes.

If you answered NO to all questions, you can be reasonably sure that you can start becoming more physically active, but please remember to build up your work-out gradually. You should also consider taking part in a fitness appraisal to determine your basic fitness.

Please remember:
- Do not exercise if you are feeling unwell due to a temporary illness such as a cold or flu. Wait until you have recovered before you resume training.
- If you are or may be pregnant, talk to your doctor before taking up exercise.
- If your health changes so that you have to answer YES to any of the questions above, tell your fitness instructor or doctor immediately: you may have to change your physical activity plan.
