If you are fully vaccinated, you will only need to submit this form once. Fully vaccinated players (older than age 12) and volunteers, will not be required to test weekly before games.

In accordance with L.A. County Guidelines unvaccinated youth (ages12 and older), and all unvaccinated staff/coaches/volunteers will require a negative test result within a 3-day window period prior to a game or competition.

Therefore, unvaccinated players (over age 12) and all volunteers NOT fully vaccinated shall fill out this COVID-19 testing compliance survey weekly in order to confirm test date and negative test results.

Local coaches and volunteers working with 12U and younger teams MUST also follow these guidelines.

PLEASE NOTE: Our regions' players age 12 and older along with their volunteers that do not have home games inside LA county will not need to adhere to this policy for games, but will still follow State/Local and Region 75 policies to help prevent the spread of Covid-19 while at Sierra Field.
