This form is for PVSD families of children in grades TK-5th (21-22 school year) that wish to enroll in tuition-based summer camp held at Monte Vista Middle School. Program hours selection are imbedded in the form. ONE FORM PER CHILD PLEASE.

Acknowledgement: Completion of this form does not guarantee enrollment. Confirmation emails will be sent out on Friday, May 13, 2022. Once families are confirmed, you will be directed to pay the non-refundable registration fee of $35. Tuition dates for summer camp are June 15th for session 1 and July 6 for session 2.
