  • The information in Section 1 will be used for the administration of the Junior Church and will be available to The Rector and the Parish Safeguarding Officer (Joy Chaplow) and will be stored securely as per our GDPR Policy
  • The information in Section 2 will be shared as above and with the Junior Church Leaders and kept with the Registers
  • The whole of this form will be made available to the main leader on any outings and visits that may occur during the year

Declaration
Whilst my child is in Junior Church I will be in St. Mary's church service. If I am unable to attend I will be responsible for appointing another adult to care for my child before and after Junior Church. That adult will inform the group leader either before the service or at the point of hand over during the service. We also understand that there is no guarantee of a first aider being present.

During the year we may attend various Outings and Visits as a Junior Church. Full details of any trips will be advised in advance on each occasion. By clicking you give your consent for St Mary’s Junior Church to take your child on organised and pre-planned Outings and Visits. You also give your consent to any medical treatment that may be necessary in the event of an emergency.
