This form is a team nomination form for the 2022 Don Green Western Junior Championships (DGWJC).

This event will take place on November 12 and 13 at Glenn Willow Regional sports Complex, Mudgee.

PLEASE ENSURE THAT THE AFFILIATE COORDINATOR ONLY COMPLETES THIS TEAM NOMINATION FORM.

Please be advised that due to restrictions on team nominations, Hornets will be approving team nominations by the following process:

- First round of teams to be assessed and potentially accepted will be all main teams from Hunter Western Hornets affiliates. These will be assessed on first in best dressed basis.
- Second round of teams to be assessed and potentially accepted will be development teams from Hunter Western Hornets affiliates. These will be assessed on first in best dressed basis.

Hornets will also be looking at accepting teams from affiliates outside of the Hunter Western Hornets region to compete if it is beneficial for the event.

PLEASE NOTE: COMPLETION OF THIS TEAM NOMINATION FORM DOES NOT GUARANTEE YOUR AFFILIATE ENTRY INTO THE EVENT.

All team nominations must be completed and received by 5pm Sunday October 16, 2022. Late nominations will only be reviewed or considered for inclusion if it is beneficial to the event.

After team nominations close, Hornets will contact all affiliate coordinators to provide the status of their nominations (accepted, waitlist, not accepted).
