SEC Confidentiality Agreement
Dear Embodied One,

At Sacred Embodiment Community, all participants are asked to sign the three following agreements before attending events.  You will sign this only once and your information will be kept on file, in accordance with our privacy policy. Please carefully read the Confidentiality, Privacy and Liability Agreements, then sign the signature sheet.  Please note that you are requested to provide full legal name and verifiable contact information (phone or email). You're welcome to ask questions and take a copy home if desired.

CONFIDENTIALITY AGREEMENT

As a participant at Sacred Embodiment Community or related events and gatherings, I agree:
- To not share about other people by name outside the community, unless I have their explicit permission to do so.
- When sharing about community members or people others might know at community events, I need to ask permission to use their name or story before sharing.
- To honor a container of confidentiality for everything personal shared at Sacred Embodiment Community, including comments and discussions on our Facebook pages, other social media and online forums.
- In conversations with Leadership or Restorative Team regarding conflict, harm or other situations where support is desired, a confidential container with the Team member will be established per conversation as appropriate. You are strongly encouraged to reach out and ask for that container to share impact or harm and be supported.
- If the confidentiality agreement at an event differs from this agreement, it will be explicitly spoken and verbally agreed to (for example, Authentic Relating events have a different confidentiality policy).  
 
PRIVACY POLICY

Sacred Embodiment Community is committed to an ethic and integrity of privacy of personal information in order to create an environment conducive to safety and trust.  In order to meet this commitment, below are our protocols for guarding personal information about participation and events in our community.

1. To protect private information of community participants, no information will be released about:
Participation at any community event or gathering, beyond the Community Director, the Restorative Team and the Leadership Team.  
Financial information or payments for events beyond the Community Director, the Financial Manager and program/event leaders.
Information given to the Restorative Team beyond the Community Director when relevant.

2. If there is a request beyond the scope of the above agreements, those requests for information must be in writing and specify the information requested, why it is being requested and how the information will be used. Release of information must be approved by the Community Director, a member of the Restorative Team and, if relevant, the Financial Manager.  The purpose of this is to ensure that a minimum of two people will be involved in the decision in order to more effectively contemplate a measured and appropriate response before any information is released.  

3. If the information for any reason would be for a legal reason, legal counsel for the community would be sought before the information is released.  
 
4. The above guidelines are effective as of July 1, 2019.  These are available in writing at any time.  They will be published on our website as soon as reasonably possible.  In addition, all facilitators for all events will have read these and have printed copies available at events, available upon request.

5. Information about event attendance relevant to the safety of participants will be shared with Community Director and relevant facilitators when deemed necessary.

 
LIABILITY AGREEMENT
I hereby agree that I am responsible for any resulting personal injury, harm, damage to or loss of my property which may occur as a result of my participation or arising out of my participation at Sacred Embodiment Community Seattle. This includes any Sacred Embodiment Community events, in-person or online. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release, indemnify, and hold harmless Sacred Embodiment Community Seattle, its officers, officials, agents, teachers, contractors, members, students and/or employees from any and all claims, demands, damages, rights of action or causes of actions, present or future.  I further state that I am at least eighteen (18) years of age and fully competent to sign this document; and that I execute this release for full, adequate, and complete consideration fully intending to be bound by the same. If I am signing for a minor (under 18), I verify that the minor is in my legal custody and I am responsible for caring for them.  

I acknowledge that I have thoroughly read this consent release and fully understand that it is a release of liability. By signing this document, I am waiving any right I or my successors might have to bring a legal action or assert a claim against Sacred Embodiment Community Seattle, the teachers, leaders and facilitators, or the Mercer Professional Building for any accident, injury or harm sustained.

I understand that I can have a printed copy of the document at any time and that questions can be answered by any member of the Leadership Team, if I have them.
