St. Patrick Family Formation 
Hello! This form allows you to register for all children and teen faith formation. 
Price for PreK to 8th formation (AMP and Encounter): $50 per child. 
Price for Confirmation Prep: $100 per child 
Price for First Eucharist/Reconciliation prep is $45

Please make checks out to St. Patrick with the program in the memo.
