OSUCOM Student Council Funding Request Form
This form allows students organizations within the College of Medicine to request funds from Student Council for OSUCOM Programming

Student Organizations will be "pre-approved" for a certain amount of funding, which will be reimbursed after submission of receipts. Requests should be filled out at least 1 week prior to the expected date of the event to ensure a timely approval.

Student Organizations will have 30 days from the time of the event to submit for reimbursement. Please email receipts to Jonathan Schmidt

Please also note that Student Organizations are capped at receiving $200 per organization per year through student council. This amount may increase pending fundraising initiatives.
