Chicago Full Moon Jam Fire Spinner 2023 Sign-Up Form
** PLEASE READ THE FOLLOWING CAREFULLY **

In order to ensure the safety of the event and all its attendees, ALL fire performers are required to attend the Full Moon Jam Level 1 Fire Safety Training. Anyone that plans to take a Fire Safety Training must sign-up in advance by filling out this form.

Once you have taken a Fire Safety Training Course, you will be added to the Full Moon Jam Approved Performer List and permitted to participate in all Full Moon Jam events for the next 3 season (end of 2025), after which, you must take the training again to remain on the Approved Performer List. If you are not sure when you last took the training, feel free to send us an email to info@fullmoonjam.org. We always send email notices to those that need to renew their training, so check your email first for "Full Moon Jam Training Expiration Notice".

We provide all fueling and safety gear as well as white gas at all Jam events, so the only thing you ever need to bring is fire-safe clothing (all cotton or natural fibers), your props, and water.

Please fill out this form to sign up to attend a Fire Safety Training and we will email you more information and the address/location of the training a couple days prior to the date that you signed up for. Be patient if you don't hear anything, we are probably just very busy.

If you are unable to attend a training that you have signed up for, you do not need to notify us and can simply attend any following training.

** All fire performers, photographers and volunteers new to the Jams must attend a Reserve Jam Training. **

All trainings begin at 6:00pm.

RESERVE JAMS
