Pixel Pals MB Incident Reporting
This form is for anonymous reporting of any incidents related to bullying, harassment, maltreatment and discrimination within NMM's community, on behalf of your self or others. 


Any behavior that contravenes the spirit of New Media Manitoba's Event Code of Conduct and Safe Space Policy won’t be tolerated. These policies can be found at www.newmediamanitoba.com/pixel-pals-mb and applies to all virtual and in-person events, all attendees/participants, staff, volunteers, organizers, speakers, partners, sponsors and vendors.

 
*** Please note: All reports, including reports regarding sexual harassment and similar incidents, will be handled with utmost confidentiality and won’t be shared with anyone outside of the Pixel Pals MB Conflict Resolution Committee/Community Safety Team. You are invited to provide as much or as little detail as you feel comfortable with sharing in the form. No action will be taken without your express involvement and consent. Thank you.



----------INCIDENT REPORTING PROCESS----------


1. Report intake received.

2. Email confirmation that report has been received within 72 hours.

3. Committee to review/discuss incident and subject of report with neutrality.

4. Reporter(s) and Subject(s) of report may be contacted if appropriate to inform them and provide additional context if available. (If this cannot be done without ensuring reporter(s) confidentiality, they will be consulted for approval before doing so)

5. Committee will convene to determine response based on available information.

6. Committee will contact reporter(s) to discuss proposed response and invite feedback before moving forward. (Reporter(s) feedback may not be acted upon but their comfort and safety are always central to this process.)

7. Subject(s) of report will be informed of the decision and what actions will be taken and/or what actions are expected of the subject(s).

8. Anonymous summary of the report, decision, and actions taken will be written and posted publicly to provide transparency and opportunity for community feedback to the incident response team.
