Dear Parents,

Please compete the following questions. The results will be used in conjunction with our Data Protection Policy.

If you have more than one child please complete a separate entry for each child.

Consent - Use of Digital Images

I have read St. Michael's Data Protection Policy (available at https://stmichaelsn6.com/wp-content/uploads/2019/10/ST-Michaels-Data-Protection-Policy.pdf) and I give the following consents:

1. I give permission for photographs/ footage to be used within school, for example on wall displays or in workbooks. *

2. I give permission for my child’s name, photograph or work or a video of such to be used on the school’s website, Google Classroom and the school's social media accounts on Twitter and Facebook. This may include photos of school trips, sporting events, concerts, assemblies or performances such as nativities or any other learning activity. When using a pupil’s image or video we will not accompany it with any other personal information to ensure that they cannot be directly identified. *

3. I give permission for my child’s image/ footage to be used in wider publications, such as the local or national press, community publications or internal borough newsletters. *

4. I give permission for my child to participate in the annual class photo. The class photograph will not be accompanied with names.
