Students: Please submit one form for each book you need.  You will be notified of the status of your request by email, so please check your email regularly.   DO NOT RENT OR PURCHASE A BOOK UNTIL YOU RECEIVE AN EMAIL FROM ME.  We may have a book available for you to check out for the semester.  If we do not have one for you to check out, you will be given permission to purchase or rent the book or e-text and receive reimbursement.  If you are approved to purchase/rent the book, you will need to return the receipt and book at the end of the semester for reimbursement.  Do NOT fill out the form if your texts are PRELOADED e-texts. See your Mr. Massey for more information on preloaded texts.  

*Textbooks are not reimbursed for summer courses.
