Intake Request Form
This form is for owners wishing to surrender their cat, kitten, dog or puppy to our adoption program. Intake requests will be assessed on a case by case basis and as space allows. Due to our kennel size and capacity, we cannot accommodate dogs weighing more than 50 pounds. 

After submitting the intake request, our staff will contact you to set up an appointment for intake consideration or give you additional resources if we cannot accommodate your pet. We are not able to accommodate pets without a confirmed appointment.
