This is an annual form where you agree to release Triple C and Triple C Farm of all liability while you are working with Triple C and Triple C Farm.

This form is effective for one year from the signing date. By filling out this form you are acknowledging and agreeing to the following:

The Staff/volunteer desires to work for Triple C and Triple C Farm and engage in the activities related to being a Staff/volunteer. The Staff/Volunteer understands that the activities include working with animals whose behavior are unpredictable, working with sick animals, some heavy lifting and working with many different cleaning techniques. The Staff/volunteer hereby freely, voluntarily, and without duress executes this release under the following terms:

Release and Waiver: Staff/Volunteer does hereby release and forever discharge and hold harmless Triple C, Triple C Farm and its Successors and assigns from any and all liability, claims, and demands of whatever kind of nature, either in law or in equity, which arise or may hereafter arise from Staff/Volunteer’s activities.

Assumption of the Risk: Staff/Volunteer hereby expressly and specifically assumes the risk of injury and/or harm in the activities and releases Triple C and Triple C Farm from all liability for injury, illness, property damage, and death resulting from the activities.

Waiver: I have agreed to provide my services to Triple C and Triple C Farm to perform various tasks as designated by the owners or management of Triple C and Triple C Farm. I covenant and agree to hold harmless Triple C and Triple C Farm or officers, directors, employees or member of Triple C and Triple C Farm from any and all injuries I may suffer which arise from my services to Triple C and Triple C Farm.

Photographic Release: Staff/Volunteer does hereby grant and convey unto Triple C and Triple C Farm all rights, title, and interest in any and all photographic images, video and audio recordings made by Triple C and Triple C Farm during the Staff/volunteer’s activities, including but not limited to, royalties, proceeds, or other benefits derived from such photographs or recordings.

I hereby understand that all the rules given to me in the orientation hold me responsible. If I do not follow them I will be asked to leave Triple C and Triple C Farm and not return as a volunteer.

**If the volunteer is under the age of 18 a parent or legal guardian must sign.**
