Welcome to another year of Faith Formation at Assumption! Our program is taking a slightly different shape this year. This form will help you place your teen where they should be. Please register one child at a time. You'll receive a prompt at the end to submit a new registration if needed.
NOTE: If your teen will be receiving Reconciliation, Confirmation, and/or First Holy Communion, you'll want to have their baptismal certificate handy.
