COVID Vaccination Status

If your child is fully vaccinated or in the process of being vacinnated, please complete our COVID Vaccination Status form. This will give us permission to review your child's COVID vaccination status in a program called the Georgia Registry of Immunization Transactions and Services (GRITS). If your child is a close contact of a school-exposed positive case, we can confirm their vaccination status without calling you or asking you to email us a photo of their card. As a reminder, students who are fully vaccinated and not experiencing any symptoms will not be required to quarantine as a close contact.
