Vacation Request Form
Family vacations and activities should be planned so as to not interfere with a student's school attendance and education.  According to the State of Wisconsin, a student can only be excused for a maximum of ten (10) days per school year. Wis. Stat § 118.15(3)(c)]

Please complete the form below for Principal approval at least two weeks in advance of the planned vacation (or as soon as possible).

Teachers do not provide 'homework' in advance of vacations. It is the student’s responsibility to complete work after their return from vacation.
