Request to open course to resolve an incomplete
In order to resolve an incomplete in Canvas a new section needs to be opened for the student and instructor. This new section will allow the student access to the course with the ability to submit work and will give you access to grade the student’s work for the time allowable based on current academic policies and procedures.

If the student needs to work with an instructor who is not the instructor of record on the original course, this request needs to come from the Dean of the appropriate school/college or the program chair.

What is the session and course ID of the course? For example, Fall 2020 ENG-101-02 *
Does a different instructor need to be added to the course? If yes, their complete email address. If not, type NO. *
What is the name and email address of the student? *
What is the date the student needs to resolve the incomplete by? *
