*current USAV membership is required - if player did not play last season (last season membership is good through 08/31/23), a try-out membership can be purchased through the Carolina Region by completing the self-guided quizzard on this site: https://www.carolinaregionvb.org/registration/ If the player is offered a spot on a team, the full membership will need to be purchased, and you will be credited the amount paid for the tryout membership when you purchase the full season membership. There are help links on the right-hand side of the registration page that will guide you through the process of first creating a Sports Engine account for the guardian and then purchasing the tryout membership for the athlete.

CONSENT & RELEASE 

I, the undersigned parent or guardian, hereby consent to my child participating in R.E.A.L. SPORTS ACTIVITIES, INCLUDING TRYOUTS, PRACTICES, CLINICS, TOURNAMENTS, CAMPS, ETC. I certify that my child is able to participate in all activities. If my child has medical conditions which may be relevant to a physician in the event of an emergency, I have listed them in the Emergency Medical Information section of this registration. In the event an emergency occurs, and I cannot be reached, I hereby authorize REAL Sports Coaching Staff to make emergency medical decisions for my child. If there are any activities I do not want my child to be involved in, I have listed them below my signature. I UNDERSTAND AND HEREBY AGREE TO ASSUME ALL OF THE RISKS WHICH MAY BE ENCOUNTERED IN ASSOCIATION WITH SAID ACTIVITIES, INCLUDING ACTIVITIES PRELIMINARY AND SUBSEQUENT THERETO. I do hereby agree to hold REAL Sports and its agents and employees, harmless from any and all liability, actions, causes of actions, claims, expenses, and damages on account of injury to my child, even injury resulting in death, or property which I now have or which may arise in the future in connection with the activity or participation in any other associated activities. I expressly agree that this release, waiver, and indemnity agreement is intended to be as broad and inclusive as permitted by the law of the State of North Carolina and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect. This release contains the entire agreement between the parties hereto and the terms of this release are contractual and not a mere recital. I further state that I HAVE CAREFULLY READ THE FOREGOING RELEASE AND KNOW THE CONTENTS OF AND ELECTRONICALLY SIGN THE RELEASE AS MY OWN FREE ACT. This is a legally binding agreement which I have read and understand.

R.E.A.L. Photo Release

In addition, I hereby grant REAL Sports to use my athlete’s likeness in photographs and/or video in any and all of its publications, including Web space, and in any and all other media.
