YOUTH CONSENT WAIVER AND RELEASE OF LIABILITY  

In consideration of being allowed to participate in any way in NSCH related events and activities, the undersigned:  

1.  Acknowledge, as the parent or legal guardian of the following minor children (homeschooled and other to be listed below) that I am signing this consent, waiver and release from liability on behalf of myself, my spouse, all parents of the minor child and the minor child.  (Attending children over age 18 must sign an adult waver).

2. Acknowledge that the minor child's participation in the activities described and/or contemplated herein are not mandatory or required as a condition of the minor child receiving other benefits and services from NSCH or participating in NSCH's educational program, and that the minor child's participation in such activities are strictly voluntary.

3.  Consent to the minor child's participation in the activities described and/or contemplated herein.

4. Acknowledge and fully understand that each minor child may be engaging in activities that may involve risk of harm or injury, including permanent disability and death,and severe social and economic losses, which might result not only from each minor child's own actions, inactions, or negligence but the actions, inactions, or negligence of others, or from the conditions of the premises or of any equipment used. Such activities include but are not limited to:: Support Group Meetings, family events, outreach events(e.g.: Enon Village, xmas caroling), clubs (e.g.:plays, chess, speech and debate), field trips(e.g.: to businesses, museums, historic sites, nature locations) , holiday parties(e.g.: Valentines), standardized testing week (testing and recess), field day (running, jumping and tug of war)and other social and educational events.

5. Assume on behalf of myself, my spouse, all parents of the minor child and the minor child, all the foregoing risks and accept personal responsibility for all harm, injury and damages arising from the activities described and/or contemplated herein.

6.  On behalf of myself, my spouse, all parents of the minor child and the minor child, release, waive, discharge, covenant not to sue and to hold blameless NSCH, its affiliated clubs, their respective administrators, directors, agents, and other employees of the organization, other members/participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors or premises used to conduct the event, all of which are hereinafter referred to as “releasees,” from any and all liability to each of the undersigned, his or her heirs and next of kin as well as administrators, personal representatives and assigns, for any and all claims, demands, suits, judgments, losses, harms, injuries and damages, and all actions and liabilities of every nature and kind whenever occurring, whether known or unknown, contingent or fixed, at law or in equity, arising from or in connection with the activities described and/or contemplated herein that each of us may suffer now or hereafter, and agree to defend, indemnify and save the releasees harmless from and against the same.
