Emergency School Closure Care NEW Registration
This is the form to continue Emergency Closure Care during the COVID-19 School shutdown. This form is only intended for previously registered families, new families must register separately.  

Emergency School Closure Care:

Dover Eyota will provide limited care to children currently attending Dover Eyota Schools Kindergarten-5th grade for children of emergency workers critical to the State's response to COVID-19 who do not have alternative care previously arranged. If possible parents should continue to use your regular childcare provider as emergency care of this kind is limited. Care will be provided only during working hours, for parents who are unable to work from home, for essential staff as listed below:

Emergency Care is a service available ONLY to parents/legal guardians who work in roles critical to patient care and the response to COVID-19. Care may be limited. Preference will be given to Tier 1 families first.

1.Complete the on-line Registration Form

2. Wait for Approval Pending from Coordinator--Jenna, pending program availability and suitability.

Children must be well enough to attend care: Sick Child: If your child becomes ill at care, the staff will notify parent/guardian immediately.  Parent/Guardian is responsible for making arrangements for child to be picked up immediately. Children should remain home or will likely be sent home if any of the following occur: Severe cold, cough, or sore throat; Vomiting, diarrhea, nausea, or abdominal pain; Temperature of 100.0 degrees or more with or without other symptoms; Eye infections, especially if discharge is present; New skin rashes, especially if discharge is present; and/or Any other signs of acute illness.  Any children experiencing COVID-19 symptoms or have a family member with COVID-19  symptoms or tests positive for COVID-19 will be excluded from care for a period of time. Written permission and signature from a licensed physician to return to care will be required. *
