Berwyn School of Music Studio Policy Agreement
To provide you with and maintain the best possible instructors, we must ask that you read and agree to the following policies.

Payment: Monthly tuition is due on the 5th of each month. Payment will be automatically deducted from your provided credit or debit card.  (Payment guarantees your scheduled day and time.) You are paying for the time slot, not the lesson. Accounts must be paid in full to receive a make-up lesson. To discontinue lessons please notify Jack Bomher or Wendy Van Sickel, with at least two weeks notice.
Cancelation Policy: If a student must cancel a lesson, you must notify  one of our lesson coordinators Jack Bomher or Wendy Van Sickel at least 24 hours prior to the lesson in order to receive a makeup lesson. Any lesson canceled with less than 24 hours notice will be determined a late cancelation and the client will forfeit the lesson. No makeup lesson or account credit will be issued for a late cancelation, or no show. Students will be allowed 6 student cancellations per year. Any additional student cancellations will be at the discretion of the instructor. Make-up lessons may be given when proper notification has been given that the lesson will be missed. In the event of a teacher cancellation, the student will be issued a makeup lesson or. If a make-up cannot be scheduled with the student's instructor Berwyn School of Music may schedule a makeup lesson with a substitute instructor.

Social Media Release: Does the Berwyn School of Music have permission to feature photos and/or video of this student on the studio website, social media accounts, and/or promotional items?

Care of Students: I understand that lessons, classes, and ensembles do not constitute daycare. Berwyn School of Music / Berwyn Recording is not responsible for providing before or after class care for students in Berwyn School of Music / Berwyn Recording. Parents with students under the age of 6 are to remain at Berwyn School of Music during in studio classes. Students over the age of 6 are to be picked up promptly after class.
Release of Liability, Medical Emergency and Injuries: As the legal parent or guardian, I release and hold harmless Berwyn School of Music, its owners and operators from any and all liability, claims, demands, and cause of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Berwyn School of Music, its owners and operators or in route to or from any of said premises. I give permission to BERWYN SCHOOL OF MUSIC, and its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian.
Parents Responsibility to be Aware of Dates: It is the responsibility of the parent or adult student to be aware of all school activities such as concerts, recitals, extra classes, and dates that the school is open or closed. Berwyn School of Music will email registered students such notices. It is the parent's responsibility to regularly check these emails to ensure that they are informed. It is the responsibility of the parents or adult students to inform the school of any address or telephone number change.
