MUSC AGM 2021 - Form for appointment of Proxy.
If you are unable to attend the club's AGM online via Zoom, you may appoint a proxy to attend and vote on your behalf. Your proxy must also be a 2021 member who is entitled in their own right to vote at a general meeting of MUSC.

By appointing a proxy, you are authorising that person to use their best judgement on all matters which come before the meeting. You can specify voting instructions for your proxy to follow if you wish.
