I hereby authorize VGWU to act as my exclusive representative in collective bargaining with my employer, Vanderbilt University, concerning wages, hours, and other terms and conditions of employment. My membership in VGWU shall be continuous unless I notify the VGWU President in writing that I intend to resign.
