Our Afterschool Club takes place on weekday afternoons when Madison and Middleton area school districts are in session and staff hours by day are listed below. Please indicate your afternoon availability! (Madison has early release on Mondays, so you'll notice two different Monday time frames.) *Please indicate all the days that you are potentially available.* *

Mondays: 12:30-2:30 pm

Mondays: 2:15-5:30 pm

Tuesdays: 2:15-5:30

Wednesdays: 2:15-5:30

Thursdays: 2:15-5:30

Fridays: 2:15-5:30

How many afternoons would you prefer to work at Club? Feel free to name a specific number or a range! *

There are multiple locations of our program. We try to take staff preference into account when creating the schedule. We have a location in Middleton at Pope Farm Elementary School and two Madison locations. There is not a busline to the Middleton location, so your own transportation is required. There are busses that come near the Madison locations. While we ask that you remain open to any location, please indicate your location preference below. *

I prefer to work at the Middleton location (Pope Farm Elementary School).

I prefer to work at the Madison locations (Randall Elementary School and Monroe Street Arts Center).

I am equally interested in both locations!

Do you have your own transportation that you can use to arrive at Club? *

Yes, I have access to a vehicle.

No, I don't have a vehicle and will be relying on bus transportation.

Creative Soul Camp Club Leaders participate in our specialty activities to set a great example for the campers, in addition to assisting our specialists and providing attentive supervision to the kids. Our specialty activities at Camp are yoga, art, culinary and drama. Is there anything we need to know about your interest in or ability to participate in these activities? *

Nope! I am able to and excited about participating and helping in all of these activities!
