Liability Waiver  I give my permission for my child to participate in FTA Program. My child and I are aware that participating in the Triton Central Youth program is a potentially hazardous activity. I assume all risks associated with participation in this sport. Including, but not limited to, falls, contact with other participants, the effects of weather, traffic, and other reasonable risk conditions associated with the sport. All such risks to my child are known and understood by me.

Payments can be mailed to Tiffani Barnhill at 8707 N Timberlane Dr. Fountaintown, IN 46130. Checks can be made out to FTA. Registration fees are $50.00 per child.
