It is the parent's responsibility to plan worthwhile objectives and activities for family sponsored field trips.  The information in this form must be completed in full by the parent. This form must be submitted five school days before the trip.  Prior approval must be obtained before absences can be approved.  The student may only have approval for one (1) trip per year and only five (5) school days will be excused unless there is an override by the Academy Director for either of these parameters.  Once this application is approved or denied by the Academy Director, the Academy Director will respond to the email address listed and copy all teachers and staff who need notification of the decision.
