Vacation Care - Permission Slip
If you would like to make a booking for Vacation Care you first need to complete this Vacation Care Permission Slip. The online permission slips can be submitted from Monday 9th September 2019.

The Vacation Care daily program which outlines the activities and costs can be downloaded from www.boosh.com.au and is available to view at The BOOSH Centre. A summary is also provided below.

Priority of Access
Families already enrolled and attending The BOOSH Centre (BOOSH families) have priority of placement. Vacation Care places can be booked by BOOSH families from Monday 16th September until all available spaces are booked.

From Monday 23rd September , any new families who have submitted a permission slip will be contacted regarding any available Vacation Care days. Any available places are offered following the Priority of Access Policy which is available to view at The BOOSH Centre or on our website.

THIS PERMISSION SLIP IS NOT A BOOKING FORM

Vacation Care days must be booked through the My Family Lounge website or app using the Casual Booking section. BOOSH families will be able to make their bookings from Monday 16th September. If you lodge your permission slip after Sunday 15th September, please be aware it can take up to one business hour for your booking calendar to appear once processed by The BOOSH Centre staff.

Once Vacation Care days are booked they CANNOT BE CANCELLED and families will be charged the full fee for the day. This includes days where the activity is changed due to weather or safety concerns. The daily Vacation Care fee is $45.00 and the total costs including any additional incursion/excursion fees are indicated on the program.
