The specific deadlines for to fill out this form are:

- Northeast: Thursday, October 10, 2019 at 11:59pm ET
- Great Lakes: Thursday, October 10, 2019 at 11:59pm ET
- Mid-Atlantic: Thursday, October 17, 2019 at 11:59pm ET
- Midwest: Thursday, October 17, 2019 at 11:59pm ET
- Southwest: Thursday, January 16, 2020 at 11:59pm ET
- South: Thursday, January 23, 2020 at 11:59pm ET
- West: Thursday, January 30, 2020 at 11:59pm ET
